Intensive case management services help people to improve their daily functioning and live independently in the community. Services include assessment, service planning, linking and referring, monitoring, advocating, coordinating services, 24/7 on-call coverage, crisis support, and skills training in the home to promote independent living, where appropriate. Case Managers assist individuals to access housing, food, medical treatment, medication management, financial assistance, government entitlements, clothing, daily living skills training, vocational rehabilitation, counseling, transportation and other available social services. Services are provided via face-to-face meetings and telephone contacts. Services are provided in the community, which may include the person’s home, place of work, treatment program, family’s home, or the provider’s office. Services are individualized and based on the person’s needs, goals, and abilities.
