The goal of mental health targeted case management is to optimize the functioning of persons who have complex needs by coordinating the provision of quality behavioral healthcare treatment and support. Services include assessment, service planning, linking and referring, monitoring, advocating, and coordinating services. Case Managers assist individuals to access housing, food, medical treatment, medication management, financial assistance, government entitlements, clothing, daily living skills training, vocational rehabilitation, counseling, transportation and other available social services. Case Management Services are provided via face-to-face contacts, telephone contacts, and collateral contacts and are provided in various places, including the person’s home, place of work, treatment program, family’s home, or the provider’s office. The specific services provided and service frequency are based on the person’s needs, goals, and abilities.
